Request Form

Friends want to help, and we need to know what you want help with. Please use the form below to tell us what you need. We will try to pair you with someone who can help you with that.  

  • The information you share will be semi-confidential.  It will be seen only by the FHF Coordinator and by the Friend(s) who volunteer to help you. 
  • If you consider your request too sensitive to be shared this way, you may speak to a member of the Ministry and Care team, or request help personally of any Friend you choose.
  • We expect Friends who request shopping or pickup to cover the cost of supplies they request. If you cannot pay, please get in touch with Ministry and Care for a potential loan or other arrangement.

Put preferred number first. Say if you are willing to receive texts.
Put one that you check regularly.
Choose your favorite from the drop-down list.
E.g.: Not after 8pm, prefer mornings, etc.
